The nomination form asks for contact information for you and the individual you are nominating for an award. Remember to get your nominee to sign for consent of the nomination.

Select an award category for you nominee
There are eight award categories. Choose the discipline of the program of study the nominee fits into or select Excellence in Teaching. The graduate and teacher awards have specific eligibility criteria, please check the criteria before you nominate. Seek permission from your nominee(s)
A signature from your nominee on the nomination form is required to confirm consent of the nomination.
The Nominee’s Story
Your Outstanding Graduate nominee’s story should be described in a short essay within the space provided in the nomination form. Give details and examples of the graduate’s education, employment success and contributions to their community.
An Excellence in Teaching nomination should include, in the space provided in the nomination form, a short essay describing the nominee, as well as examples of their teaching accomplishments, relationships with students and contributions to their community.
Provide a photo of the nominee
We require a studio-quality photograph to include in our onsite awards program, as well as for material publicizing the award winners. The photograph must meet the following criteria:
Size: A minimum of 1800 x 2400 pixels, or an original 5” x 7” at 300 dpi unedited.
Format: .jpg only
Portrait orientation, head-and-shoulder shot only
Award nomination deadline
The completed nomination form is to be received no later than April 15, 2019.
Send it to us
Complete nomination packages will be accepted by email, and all completed forms must be typed within the provided Microsoft Word document unless otherwise requested (ex: providing additional hard copies as per the directions in the nomination form).
If you cannot include a digital signature with the document, submit the completed digital nomination in a word document along side a scanned copy with the appropriate signature.
CCO will not accept scanned or printed nominations without an accompanying Microsoft Word document.
